To successfully submit your Coopervision rebate, you'll need to:

Gather your documents

You will need the following documents to complete your rebate submission:

Original dated sales receipt with eligible lens purchase(s) and purchase date circled.
Original dated eye exam receipt with eye exam and date circled.
Correct number of ORIGINAL end panels with prescription information from product boxes. You are required to submit the quantity of end panels equaling the qualifying number of boxes purchased to be eligible for the rebate. For ClearSight^TM 1 day toric and Proclear^® 1 day multifocal ONLY you are required to submit half the number of end panels as boxes purchased.

Prequalify online

Complete the online prequalification process and print your rebate submission confirmation page.

To begin the online prequalification process, please enter the Code found on the back of the rebate form you received from your Eye Care Professional, then click NEXT below.

Mail in your documents

Mail all required documents to the address provided on your confirmation page.

Read all instructions carefully to ensure there are no delays in processing your rebate submission. Please make a photocopy for your records.

CooperVision^TM Rebate Offers

If you’ve recently had an eye exam and purchased eligible CooperVision lenses, follow the steps below to receive your CooperVision Visa® Prepaid Card by mail.

1) You will need the following documents to complete your rebate submission:

Original dated sales receipt with eligible lens purchase(s) and purchase date circled.

Original dated eye exam receipt with eye exam and date circled.

Correct number of ORIGINAL end panels with prescription information from product boxes.

Rebate form for your CooperVision lens product (provided by your Eye Care Professional).

2) Complete the online prequalification process and print your rebate submission confirmation page.
Follow the instructions to complete the 5-step online prequalification process by entering the required information. When finished, print the confirmation page to mail in with the other required documents.

Please note: Submissions must be postmarked within 60 days of lens purchase. Contact lenses must be purchased within 90 days of your eye exam, and all receipts must be from the same eye care practitioner who prescribed your contacts, or from a location affiliated with that practitioner.
